Supracondylar Humerus Fracture
The most common pediatric elbow fracture, usually from a fall on an outstretched hand, with risk to neurovascular structures.
Presentation
- Painful, swollen elbow after fall, refusal to move
- Assess anterior interosseous nerve and radial pulse carefully
Imaging & Workup
- AP and lateral elbow — anterior humeral line, posterior fat pad
- Gartland classification (I–III) guides treatment
Treatment
Casting
Type I (non-displaced) in long arm cast.
CRPP
Closed reduction and percutaneous pinning for displaced types II–III.
Clinical Pearls
- Pink pulseless hand requires urgent reduction and vascular assessment
- Watch for compartment syndrome
